Neighborhood Association - Directory Listing Form

All neighborhood residents are welcome to be listed in the University Hill Farms Directory.

During the annual membership drive, you will have the opportunity to update your household information. Please check your current directory listing to see if the information is correct. If there are any changes or corrections, please give the updated information to your block captain during the membership drive.

Youths looking to do yard work (lawn mowing, snow shoveling, etc) during the year are encouraged to add a "#" symbol by their name in the directory. Similarly, youths looking for babysitting work should add a "*" symbol by their name.

If you have directory changes and have not contacted by your block captain by October 1, please fill out this form and return it to the board member in charge of membership and directory, Elizabeth Katzelnick, 4910 Fond du Lac Tr. 53705. She can also be reached at betsy@hillfarms.org.

Please do not submit a form if the current information is still accurate.

A new directory is produced each year, and directories are distributed to paid-up members in January.
